Dialysis Social Worker (MSW) needed at our Cornerstone Dialysis in Southfield, Michigan.
Schedule: Full time, Monday - Friday (8hrs/day)
Case load: 125 patients (split between in home and in center dialysis patients)
Training: 3 weeks of training
In this vital role, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease. This can include crisis counseling, mental health services, and connecting patients with community programs and resources.

Requirements of the position:
- We are looking for someone with a ton of energy
- Master's degree in Social Work (MSW)required
- Licensed or registered in the state of STATE as required by state regulations - list state license types here
- Demonstrated knowledge of government and private insurance programs
- Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
- Dedication to caring for patients suffering from chronic kidney failure.
