Position Summary

Responsible for the organization of patient care for patient with suspected sepsis or confirmed sepsis. Coordinates patient care services with an emphasis on clinical guidelines and outcomes management. Sepsis coordinator leads, directs and negotiates through collaboration with physicians and staff to promote care delivery that assures efficiency / clinical outcomes, and cost effectiveness at entity. To maintain the nursing staff in all areas as well as to deliver safe efficient age specific nursing care to patient/families.

  • Program development, establishes and monitors care standards and ongoing program maintenance. Coordinates, evaluates the effectiveness care for stroke.
  • Designs teaching programs for patients, families and provides assistance to nursing staff in program implementation
  • Acts as sepsis resource for nursing and direct care providers, implements and enhances patient education activities
  • Provides specialized nursing knowledge and expertise functioning as clinician, educator, researcher and consultant to medical / nursing staff, patients and families
  • Oversees sepsis program development in conjunction with Medical Director and CNO.
  • Develops policies and procedures, facilitates performance improvement teams for sepsis.
  • Evaluates and ensures compliance with all accrediting agencies regarding sepsis.
  • Performs daily and concurrent reviews of patients treated with sepsis protocol for adherence, performance and outcomes
  • Collects data and abstracts and enters data for sepsis and analyzes data and recommends action plans to team
  • Represents sepsis care area of specialization on departmental and hospital task forces
  • Assures delivery of "Best Practice Standards" for sepsis care and participates in program education, outreach and research activities
  • Develops effective presentation tools to facilitate, interpret, and analyze sepsis
  • Facilitates the sepsis interdisciplinary meetings, develops agendas and quality review processes with medical directors and CNO.
  • Collaborates with marketing to develop community outreach / educational programs
  • Actively participates in sepsis performance improvement program including data analysis, medical record extrapolation and data entry
  • Identifies in collaboration with CNO relevant areas for nursing research and grant proposal writing
  • Assuring compliance with regulatory bodies
  • Monitoring tracking entry analysis data interpretation
  • Education for physicians, nurses, hospital staff and community
  • Action Planning for Sepsis
  • Research and grant proposal writing
  • Will cover both Tripoint & Beachwood Medical Center
  • Sepsis & COPD Coordinator
