The Patient Coordinator Clinic Navigator will function as a patient coordinator telephone resource to Summit patients and serve as an extension of, and a conduit to, all service line patient care teams. This position will be responsible for receiving incoming calls from Summit patients with the goal of successfully addressing their questions or needs, updating information in the patient record, and escalating those that require further patient care action from the respective service line care teams.

This is a full-time role that will primary based at our Corporate Office in Woodbury. Scheduled day hours are Monday - Friday, 8:00am- 4:30pm.

Flexibility to float to other sites as needed.

Job Responsibilities:

  • Responsible for answering and responding to patient phone calls for all service line care teams on a daily basis.
  • Accountable for accurately assessing patient inquiry or need, providing assistance for initial resolution. When inquiry requires clinical intervention or response, determine appropriate course of action and escalate to care team or other Summit resources, as necessary.
  • Responsible to support coordination of patient care between team members, other service line teams and/or other Summit departments. Communicate information from patients to physicians and other care providers in support of patient care. May provide information, complete or update standard (non-complex) paperwork requested by patient as directed by physician or other care provider, and/or in accordance with established physician protocols.
  • Accurately enter information to service line team tasks lists, when appropriate and enter physician's orders in to electronic medical record.
  • May act as back-up to other physician teams, assisting with voicemail, email and task lists as needed; proactively reaches out to offer assistance.
