Scope of work

Responsible for the care management of Oakwood Common residents and the administration of the Lifeline System. Conducts routine assessments of residents and/or when requested by others. Maintains a general understanding of the health of the residents. Keeps physician and family members current on problems or changes in a resident's health conditions. Coordinates and evaluates resident incidents, education and wellness programs. Assures that residents receive home care service or courtesy observations when they are experiencing short-term illness. Provides nursing services to residents for their routine health needs. Responsible for assessing ancillary charges for all related services. Assists Clinical Care Coordinator/Nurse Manager in all areas. Creates and maintains medical records for residents and assures availability to health practitioners who visit residents in the Wellness Center.

Qualifications

  • Required Associate's Degree or equivalent Graduate from an accredited school of professional nursing as a Register nurse or Licensed Practical Nurse with an associate's degree or higher.

  • 3 years of relevant experience 3 to 5 years in geriatric health, public health, emergency room or independent practice, required

  • Registered Nurse (RN) - State of Michigan Or License Practical Nursing (LPN) - State of Michigan, required
