This role will be with our Stabilization and Mobile Response Team in the Western Region. It is an as-needed position where the SSW will be expected to work an average of one twelve hour shift per week. The Stabilization and Mobile Response program is available to children/families 7 days-a-week, including weekends/holidays. Our Stabilization and Mobile Response Team is dedicated to helping children and families, in their homes, to address challenges and to provide support. Training will take place in Taylorsville, at the Wasatch Canyons Campus, but the office space for this position is anticipated to be set up in Utah County after initial training. The SSW hired for this role will be triaging phone calls coming in, assisting in virtual and in-home mobile response visits for children/families, and assisting with in-home, short-term, stabilization services. During Mobile Response visits, the SSW and team focus on: assessing the child/family/situation; creating safety plans; working with the family to minimize the impact of the challenge they face; connecting the child/family with the appropriate resources. During Stabilization Service visits, which is a short-term in-home program for children/families, the SSW and team focus on: teaching skills to improve family functioning; creating plans that prepare the family for and to prevent future challenges; preventing the need for out-of-home services; equipping the family with ongoing resources and support.

This position provides care appropriate to the established plan of care in the assigned patient population working at the Utah Valley Clinic.

Job Essentials

  • Organize and prioritize daily work by reviewing new, current, and discharging patient needs in area(s) of responsibility. Complete documentation as required.
  • Gather psycho-social information for patients and caregivers.
  • Educate, communicate, and coordinate with multiple patient care teams and outside agencies as necessary to ensure the patient receives quality care, a timely intake and discharge, and to identify and resolve delays and issues.
  • Acts as an advocate on behalf of patients and families, where appropriate, to ensure effective communication with multidisciplinary care team, outside agencies and other resources as necessary.
  • May provide initial intake, crisis intervention, and adjustment to illness counseling services as well as appropriate referrals to treatment and/or resources.
  • May identify patients at risk for potential avoidable delays, extended stay, readmission, financial or complex discharge needs. Coordinate with care providers and leaders to reach optimal solutions.
  • May maintain an active role in denial prevention. Proactively intervene with payers to prevent inpatient denials. Communicate any necessary information to payers to help appeal existing denials.
  • May coordinate the discharge planning process from patient admission to departure. Screen for and identify patient needs, develop a discharge plan, and coordinate with internal and external services for care during and after the hospital stay.
  • Provide support and education to patients, families and staff members on Advance Directives.


Minimum Qualifications

  • Bachelors degree from an accredited institution (degree will be verified).
  • Current Utah state licensure as an SSW
  • Computer skills (email, word processing, and spreadsheets).

Physical Requirements:

  • Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
  • Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).
  • May be expected to stand in a stationary position for an extended period of time.
