Job Summary

The Community Healthcare Advisor provides outreach and enrollment services to Denver Health patients, families and communities who may be uninsured and eligible for insurance programs. Assists identified clients in scheduling an enrollment appointment, prescreening clients prior to the enrollment appointment, collect required enrollment documentation, fill out applications, and additional tasks assigned. Additionally assists with community outreach events as needed. Participates in outreach events and provide information, resources, and education regarding health insurance, and accessing services.

Essential Functions:

  • Assist clients in-person and/or via calls to schedule for an enrollment appointment and pre-screen clients accordingly. (40%)
  • Collect all required documentation and complete application prescreening process, including copies of documents, placement of attest labels, and completion of application for Medicaid, CHP+, CICP, and DFAP. (40%)
  • Participates in outreach events and provide information, resources, and education regarding health insurance, and accessing services. (20%)


Education:

  • High School Diploma or GED Required


Work Experience:

  • 1-3 years community involvement experience Required


Licenses:

  • BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association Required


Knowledge, Skills and Abilities:

  • Bilingual English/Spanish, preferred.
  • Direct, empathetic, effective client or customer service.
  • Experience in community service or customer service work or volunteering.
  • Demonstrated experience in developing and maintaining excellent rapport with clients and staff
  • Communication and motivational interviewing skills
  • Ability to maintain professional demeanor when dealing with difficult individuals and situations.
  • Skilled in the use of Microsoft Word, Outlook, Excel, PowerPoint
  • Epic ADT/Cadence Knowledge, preferred
