Care Coordinator -Center for Surgical Optimization (CSO)

Hours: Part-time (40 hours bi-weekly) - 8:00 am-4:30 Monday -Friday No weekends or holidays

Location: Wilmington Hospital

The Care Coordinator for the Center for Surgical Optimization is seeking a Registered Nurse to manage the care of identified patients undergoing a joint replacement in conjunction with the patient's primary physician/surgeon and family member, according to

agreed-upon clinical guidelines, to coordinate high-quality patient care across all care delivery sites.

Responsibilities:

Completes and documents a preoperative health assessment screening and perioperative history and assessment for patients scheduled for joint replacement surgery according to established criteria.

Reviews clinical documentation and communicates findings with providers of the CSO Team and attending physician/anesthesia team and revises plan of care as needed based on findings.

Demonstrates skills and knowledge necessary to provide care to adolescent, adult and geriatric populations, including knowledge of growth and development, the ability to obtain and interpret information to identify patient needs and to provide the appropriate care needed.

Assesses patient/family learning needs/barriers and carries out teaching measures and makes all appropriate referrals utilizing available resources.

Completes follow-up phone calls within 48-72 hours of patient discharge and reports findings, contacting surgeon with questions or concerns.

Demonstrates ability to prioritize workflow to complete tasks.

Establishes methods that support reciprocal communication between physician offices, lab services, specialty services, and the CSO team.

Assesses patient post-discharge care needs, initiates plan of care, and initiates inclusion of the necessary interdisciplinary team members.

Communicates and coordinates with patients concerning physical therapy and DME needs in preparation for post-operative needs.

Participates in system-wide care management and other program activities as requested. This includes rounding on 7S joint replacement patients for discharge preparedness, multi-disciplinary rounds, and updating tracking forms.

Maintains collaborative professional working relationships with clinicians and administrative staff necessary to develop and implement a successful cross-continuum care management for patients undergoing joint replacement.

Collaborates with the Total Joint Replacement (TJR) Program Manager on ongoing performance improvement initiatives.

Demonstrates understanding and application of CCHS values and behaviors in daily work.

Serves as a patient advocate when personal/political issues impede progress and/or collaboration between practitioners involved in care that is fragmented.

Performs patient education in person, via video, or through telephone using teach-back principles; collaborates with patient and family to establish realistic goals and activities to enhance patient self-management and participation in plan of care through discharge.

Uses theoretical and educational frameworks and expert clinical knowledge base to promote, develop, and provide patient and family education.

EDUCATION AND EXPERIENCE REQUIREMENTS:

RN, licensed to practice nursing in the state of Delaware.

BSN required Masters degree in Nursing preferred.

BLS required

Three or more years of clinical experience in acute care or Perianesthesia care required

Relevant, direct orthopedic experience as a professional nurse

Approved certification preferred, or to be obtained within two years of hire into this position.
