LSSI is hiring a full-timeOutreach Worker.Identify, outreach, and engage residents at Chicago area SNF & SMHRFs, providing education and information on the Williams/Colbert Consent Decrees and the scope of services available post-transition.

EssentialFunctions:
  • Engage Class Members and build rapport w/staff at nursing home setting, review chart at nursing facility, collaborate w/ providers, family, & guardians.
  • Aware of accreditation and licensure standards; adherence to ethical standards to treatment according to NASW code of ethics.
  • Complete documentation according to the Williams & Colbert Consent Decree requirements and direction provided by IL Dept. Human Services.
  • Engage in individual and group supervision with supervisor.
  • Educate Class Members, staff at SNF/SMHRFs, guardians/family/significant collaterals about Colbert/Williams Consent Decree and available supports.
  • Completion and timely submission of all required documentation toIL Dept. Human Services.
  • Provide groups to Class Members, NH staff, guardians, families or with significant collaterals.
  • Demonstrate sensitivity and responsiveness to cultural differences present in the agency's service population.
  • Comply with productivity standards set forth by agency and DHS.
  • Performs other related tasks as needed.
Position Qualifications:
  • High School diploma or GED; customer service experience preferred.
  • Ability to transport self to nursing facility, LSSI sites, and meetings and conferences as scheduled.
  • Valid Illinois Driver's License and insurance required.
  • Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office - Outlook, Word, Excel and PowerPoint) along with other related software.
