PRIMARY FUNCTION: Responsible for coordination of services for children with severe emotional problems and their families.


PRIMARY DUTIES:

Legend: E = Essential / NE = Non Essential / N/A = Not Applicable

BILINGUAL POSITIONS MUST BE ABLE TO PERFORM ALL OF THE DUTIES LISTED BELOW IN ENGLISH AND SPANISH:

1. Facilitate all aspects of services for participants (transport participants, assemble child and family team, assess needs and strengths, develop an Individualized Recovery Plan (IRP), complete functional screens on a yearly basis, advocate, provide or secure services (including transportation), testify in Court as required, etc.). E

2. Serve as a liaison between the child/family and community service providers and assure acquisition of services. E

3. Monitor and negotiate provision of services, and agreements, through child and family team, modify the IRP as needed and SAR when new services are added and/or existing services discontinued. E

4. Prepare, complete and submit required items by due dates (Progress Notes, IRP, Crisis Plan and Service Authorization Request (SAR), measurement instruments, Court required documents, etc.). E

5. Establish and maintain good communication, collaboration and cooperation with all stakeholders (i.e., BMCW, probation workers, etc.). E

6. Follow legal, organizational, and contractual laws and policies. E

7. Attend all meetings and professional development activities and perform presentations, as directed. NE

8. Follow all duties of a mandated reporter. E

9. Perform other related duties, as assigned. NE

10. Be a team player. E

Requirements

PHYSICAL REQUIREMENTS:

Regularly required to drive, stand, sit, reach, stoop, bend, and walk. Infrequent lifting may be encountered.


WORKING CONDITIONS:

Work is generally performed in field and office settings. Frequent local travel; some state-wide travel may be needed. Flexible hours required; as dictated by program needs.


REPORTS TO: Program Coordinator, as designated


MINIMUM QUALIFICATIONS REQUIRED:

1. Bachelor Degree from an accredited school in Social Work or related field (i.e.; psychology, sociology, counseling).

2. One (1) year continued experience providing mental health services.

3. Bilingual, Spanish and English, highly desirable (required for bilingual positions).

4. Strong communication skills, cultural competency, and interpersonal relationship skills.

5. Excellent critical thinking, highly developed organizational skills, and able to manage multiple priorities.

6. Able to work and communicate frankly and effectively with adults, families, and all team members to implement problem solving strategies, understand issues, and to provide crisis services in an ethical, respectful, responsible manner.

7. Knowledge and experience with experience in using Microsoft Office suite.

8. Able to work flexible hours, including evenings and weekends depending on needs of the youth/family and program needs.

9. Transportation, valid Wisconsin Driver's license, state minimum auto insurance, and must meet La Causa, Inc. driving standards.

10. Must successfully pass all background checks.
