Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking a qualified Rehabilitation Counselor to provide support, education, and guidance to individuals who have been identified as having a mental health and personality disorder and involved with the criminal justice system.

Additional Responsibilities include:

  • To assist with participant activities, skill building, goal achievement and local community interaction.
  • To implement the overall mission, values and beliefs of Horizon House in all aspects of the job.
  • Assist with the growth and development of the participants including skill building and social support utilization in accordance with the principles of Dialectical Behavioral Therapy.
  • Provide one-on-one behavioral intervention in the home or community settings in accordance with strategies identified in the individualized treatment/service plan.
  • Participate in developing goals with the participant, for inclusion in the individualized treatment/service plan.
  • Maintain and submit required program reports and documentation, such as progress notes describing the contact and how it relates to treatment goals, plan reviews, incident reports and medical/appointment logs.
Job Qualifications:
  • Bachelor's Degree in Human Services/Social Services, Healthcare or related field with least 1 year of experience working directly with individuals with mental health and/or other behavioral health disorders or in a forensic setting; or Associates Degree, or 60 credits in human services or related field and 3 years experience in working directly with individuals with mental health disorders and/or other behavioral health disorders or in a forensic setting; or High School Diploma/GED and a minimum of 5 years experience in mental health and/or other behavioral health disorders or forensic setting.
  • Experience in residential or other services to individuals with Severe Mental Illness or Personality Disorders preferred.
  • Good communication and counseling skills.
  • Good writing and computer skills for documenting in an electronic health record.
  • Ability to work independently and as part of a focused team.

Scope:

  • Frequent talking and listening.
  • Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with participants.
  • Will require crisis intervention in emergency situations, including managing aggression.
  • Possess manual dexterity and fine motor skills.
  • Possess a valid driver's license and an acceptable driving record.
  • Local travel required, including transporting program participants.
  • May be required to report to work during emergencies, including inclement weather.
