Summary

JOB SUMMARY: The Nurse Navigator - Cancer Institute will provide care coordination services for the Medical Oncology department at Sinai Hospital of Baltimore. Following a collaborative team model, the Nurse Navigator identifies and manages high risk patient populations, coordinates their care, and provides safe, age appropriate patient care. In conjunction with provider team, identifies patients at risk for poor outcomes. Provides proactive outreach, including telephone and face to face interaction in the clinical setting. In collaboration with physicians, nurse practitioner, and other members of the treatment team, develop individualized care plans for patients specific to the patient's needs. Facilitate communication between the patient, family, and other members of the treatment team.
REQUIREMENTS: BSN degree and 3-5 years experience required. Oncology certification required (OCN, APHON or other ONCC cert). Outpatient setting experience preferred; Maryland Registered Nurse License (RN); American Heart Association CPR Certification (BLS).
