Job Description


JOB SUMMARY: Promotes LifeBridge Health Post Acute Services to in-system hospitals as well as external facilities, patients, families and providers. Completes patient assessment to determine level of care needs and recommends admission to appropriate post acute services. Serves as a liaison between patients' current provider and LifeBridge post acute services, working to convert patients for admission. Responsible for marketing LBH post acute services to assigned territory.

Essential functions include:
Evaluation of patient referrals from physicians, social workers, case managers and family members for potential admission to LifeBridge post acute facilities.

Marketing of LBH post acute services.

Communication with staff at referring institutions, patients, families and staff at Levindale, Northwest Subacute, Sinai Acute Rehab or other assisgned locations/units to provide the patient a smooth

REQUIREMENTS: Associate's degree required; Bachelor's degree preferred. Clinical background as a RN, LPN, RRT, PT or other related clinical licensed profession is preferred. 3-5 years of experience. Valid driver's license.
