40-hour temporary social work position to provide a range of clinical social work services and case management to the outpatient Complex Care Service. The CCS provides services and care to patients that are significantly medically compromised.
This Temporary Outpatient Clinical Social Complex Care Service will be responsible for:
Psychosocial screening and assessment, counseling, and crisis intervention.
Facilitation of use of hospital and community services, resource information and referrals.
Program development and psychosocial consultation to multidisciplinary team.
To qualify, you must have:
Master's degree in Social Work. LCSW or LICSW licensure in the state of MA.
Flexibility and the ability to work in fast-paced, dynamic environment.
Excellent oral, written and interpersonal communication skills.
Experience in medical environment and bereavement highly preferred.
