Description

Lutheran Social Services of WI & Upper MI is currently seeking a part-time, 20 hour per week, Case Manager to join our housing team. This position is a part time case management position located in Racine and Kenosha county. The case manager in this position also acts as a housing resource specialist and provides supportive services including but not limited to life skills training, employment or income assistance, educational assistance, and referrals. Knowledge of resources available for this population is a plus.

Under direction, delivers professional level social work case management services to individuals and families; and performs other duties as required.


ESSENTIAL DUTIES AND RESPONSIBILITIES:
This list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time.
* Markets services and/or receives referrals of customers with a variety of needs which require the application of professional social work counseling skills to solve.
* Conducts assessments of customer needs through interviews, collateral contacts and other investigations, develops a casework plan with customers, and performs agreed upon and necessary casework services.
* Provides case management services, and coordinates the various social, community, and mental health services received by customers.
* Establishes and maintains effective public and working relationships and collateral contacts.
* Maintains detailed case records, develops and prepares statistical reports, makes recommendations concerning case disposition, and testifies at hearings as required.
* Attends and participates in staff development programs, including in-service training, staff meetings, and professional seminars.
* May provide consultation to other social work staff and para-professionals.
* May serve as a representative of the agency to interested individuals, professionals, and community groups.
* May administer specialized programs within a community or the agency.
* Participates as a member of a quality team and interacts with primary, external, and internal customers of the agency in such a way that the customer experiences increasing quality.
* Performs other duties as required.


QUALIFICATIONS

EDUCATION AND/OR EXPERIENCE:
* Bachelor's degree in social work, sociology, psychology or related field or an equivalent combination of education and/or experience is required. May be dependent upon COA and/or contract/licensing requirements.
* In addition to the above, two (2) years of professional work experience.
* Comprehensive knowledge of counseling and casework principles and methods.
* Considerable knowledge of welfare, health, mental health, and community resources.
* Considerable knowledge of the federal, state, and local rules and regulations governing the delivery of social services.
* Considerable knowledge of human behavior, dynamics of groups, interpersonal relations, and social interaction.
* Considerable knowledge of social problems and their effects on individuals.
* Ability to provide professional social work treatment to customers.
* Ability to organize and plan work effectively.
* Ability to establish and maintain effective public and working relationships.
* Ability to prepare and present detailed and complex reports.

Housing Resource Specialist:

Essential Duties and responsibilities:

  • Assist participants in locating, securing and maintaining housing of their choice through the strength based model and program guidelines.
  • Act as liaison between property managers/landlords and participants.
  • Ensure rental properties meet Housing Quality Standards and are appropriate for participants.
  • Calculate, collect, and record rent.
  • Generate and enforce leases as required by state law and program requirements.
  • Provide a comprehensive strength based approach to supportive services.
  • Maintain participant case files confidentially and in compliance with contract and agency requirements.
  • Collect and report program data; input data into Homeless Management Information System (HMIS), and Evolv.
  • Provide participants with program parameters; explain process and answer any questions.
  • Review participants responsibilities within program.
  • Offer problem resolution options for participants with landlords.
  • Facilitate supportive service activities for participants that promote achievement of goals and objectives, utilizing strength-based housing plan.
  • Provide advocacy for participants; support, problem resolution, and identify resources.
  • Offer support and solutions for grievances brought by participants; Partner with Program Supervisor and/or Program Manager.
  • Attend any additional training necessary to stay compliant in program.
  • Facilitate group events, trainings and consulting to participants to support income and self-sufficiency goals.
  • Remain up-to-date on grant guidelines, renewals and standards to ensure timely implementation of changes.
  • May facilitate family meetings with clients and involved family members for assessment and planning.
  • May monitor the ongoing provision of services from community agencies and keep the case management and provider agency current with the progress of the individual.
  • Establishes linkages with all agencies and service providers in the community; determine the best service to assure individualized, quality, flexible and creative services for the involved client(s).
  • Establish and display on site community resource information for services such as case management, personal assistance, homemaker, meals on wheels, transportation, counseling, occasional visiting nurse, preventative health screening/wellness and legal advocacy.
  • May coordinate volunteer support programs with service organizations in the community.
  • Maintain schedule flexibility as needed for case management meetings.
  • Coordinates wellness/education sessions to clients as needed.
  • Demonstrate the ability to work with the Housing Services team, the agency and the community in a positive manner as well as the ability to work independently.
  • Working with Program Supervisor and/or Program Manager, establish program specific outcomes, data collection tools and implement in program.
  • Gather data and prepare required reporting including, but not limited to reports required by HUD and the Continuum of Care.
  • May be required to give oral reporting to governing organizations.

CERTIFICATES, LICENSES, REGISTRATIONS:
* Must have a valid driver's license and have reliable transportation to perform the essential duties of the role; a motor vehicle check (MVR) with a satisfactory driving record per the LSS Driver Safety Procedure is required, and ability to meet LSS auto insurance requirements.* State Certified Social Worker (CSW), Certified Professional Counselor (CPC), or other applicable state certification is preferred.

TRAVEL: Ability to travel on day trips as required up to 80%. Some overnight travel may be required.
