Services – With 5 bilingual medical and behavioral health clinics, 23 school-based clinic sites, and a satellite geriatric clinic and early learning center, we provide:

  • Primary medical care

  • Behavioral health care
  • Substance use disorder services
  • Chronic conditions education (i.e., asthma, diabetes, hypertension)
  • Social services
  • Prenatal case management
  • HIV services
  • Women, Infant & Children (WIC) Nutrition Program
  • Environmental Health and Community Wellness initiatives (lead outreach, habitat restoration, healthy choices programs)
  • Oral health care
  • Specialty care & PT/OT

Impact - Through our medical and behavioral health care, support services, and community outreach programs, we care for individuals and families who are typically difficult to reach. In 2021:

  • 41,545 patients served
  • 85.8% identify as Hispanic
  • 70% prefer care in a language besides English
  • 74.8% live at 100% of the poverty level or below
  • 19.3% are uninsured
  • 58.8% are on Medicaid
  • 42.6% are under 18 years of age
