HISTORY Progressive Community Health Centers, Inc. was created in April 1999. It was incorporated with the mission of providing health care services to the residents of the inner-city west neighborhood of the City of Milwaukee. The two health centers, Lisbon Avenue Health Center and Hillside Family Health Center are uniquely able to assess the major health problems of the population they serve. They accomplish this through their presence in the community, expressed by leadership and participation, advocacy, linkage with its neighborhoods, ongoing networks with other providers and cooperation with city, country and state departments of health and related services.

SERVICE AREA Progressive Community Health Centers, Inc.'s service area is home to approximately 70,000 persons, over 72% of whom live below the Federal Poverty Level. The service area is defined by 36 census tracts, situated in the western section of the inner city. Of these census tracts, 33 are designated as Medically Underserved Areas (MUA's) and 13 of these tracts are defined as primary care Health Professional Shortage Areas (HPSA's). The Health Center ensures availability and access to all patients through the utilization of a sliding fee scale and a commitment to servicing the underserved and uninsured. This model/structure ensures a cost-effective, comprehensive approach to meeting the primary care needs of the target population. It also assures a seamless continuum of care and access to appropriate specialty care for the target population.
