Lutheran Social Services of WI & Upper MI wants you to join the team at Homme Youth and Family Programs, a nationally known treatment program serving children and adolescents dealing with issues of sexual perpetration, delinquency, victimization, AODA, cognitive behavioral issues or dual diagnosis, and independent living support.
The Youth Residential Nurse has day-to-day responsibilities for one or more practices within the residential program within the Business Unit. Responsibilities include but are not limited to, effective coordination of day-to-day program operations, direct supervision and performance management of staff and their work assignments, ensuring the program meets licensing and/or contract requirements, ensuring agency best practice standards are met.
In addition, the Youth Residential Nurse, consults, assesses plans, evaluates, and administers skilled nursing care to clients and performs other duties as required. The position works collaboratively with staff involved in medication management and filling of psychotropic prescriptions for youth in out of home care.
Essential Duties and Responsibilities:

- Serves as a representative of the agency to internal and external customers regarding the work of Homme programs and services.
- Trains LSS employees who complete medically related tasks, as a requirement of their job. Ensures program integrity and continuous improvement in these areas.
- Participates as a member of a team consulting in areas of treatment, medication, and health/wellness of clients. Attends weekly meetings with medication prescriber.
- May administer medications and treatments as prescribed and observes for cause and effect.
- May assess clients upon admission or as needed throughout the individual’s stay, identifies problems, and may recommend initial phase of treatment plan.
- Provides oversight and consultation for individuals dealing with mental health issues and/ or physical disabilities.
- Documents nursing intervention in Evolv and client response and communicates information to involved staff.
- Maintains accurate, complete records of client mental and physical conditions, nursing care provided, and medication and treatments administered.
- Maintains client confidentiality and provides a safe and secure environment.
- Participates in educational programs to remain current with changes in health care.
- Participates as a member of a quality team and interacts with primary, external and internal clients of the agency in such a way that the client experiences increasing quality.
- “On call” – Available to take calls related to medication questions, medical issues, communicable diseases, and serve as a resource person for staff during off hours.
- May supervise medication administration and ensure programs are following all required guidelines, best practice, and following proper procedures.
- May provide trainings in such things as proper medication administration, diabetes training, and other topics as needs arise in the program.
- May delegate additional training beyond the state approved medication training that allows staff to provide such things as injectable or nebulizer medication with proper training, skill demonstration, and ongoing supervision.
- May provide on-site reviews of medications monthly, quarterly and annually as dictated by all required regulations.
- Reviews medication errors to identify trends, needed improvements to process, etc.
- In collaboration with HC and others as needed, may provide basic consultation related to occupational health matters such as infectious disease prevention, post- injury assessment, first aid care, and appropriate occupational medical follow-up after staff return to work.
- Performs other duties as required.

Qualifications

EDUCATION AND/OR EXPERIENCE:
- Completion of an educational LPN program and credentialed as a NCLEX-PN is the minimum educational requirement.
- Requires a minimum of 1 year of experience working as a Licensed Practical Nurse (LPN) or Registered Nurse (RN), including knowledge of the modern principles, practices, and techniques used in assessing, planning, implementing, and evaluating nursing care.
- Working knowledge of the physical and psychological behaviors and needs of clients.
- Working knowledge of the legal aspects of nursing and how the laws relate to clients.
- Ability to plan for, participate in, and evaluate total nursing care. This includes critical thinking skills related to assessing individual health needs, symptoms, and behavioral changes to determine when action is appropriate.
- Experience with medical procedures deemed essential to this position, which may include medication administration, giving shots, maintaining records, providing personal supports, and others as necessary.
- Ability to complete detailed records and reports accurately.
- Ability to establish and maintain effective public and working relationships.
CERTIFICATES, LICENSES, REGISTRATIONS:
- The incumbent of this position must have a valid driver’s license and have reliable transportation to perform the essential duties of the role; a motor vehicle check (MVR) with a satisfactory driving record per the LSS Driver Safety Procedure is required, and ability to meet LSS auto insurance requirements.
- Current license to practice as a nurse in the applicable state as a LPN or RN and has current NCLEX-PN or NCLEX-RN.
- Current CPR certification.
