Description

Midway Crisis is a 4-bed Adult Family Home (AFH) for crisis stabilization in Eau Claire County. This program serves adult men and women with mental health and/or AODA issues to provide respite in a crisis situation. These crises may include loss of housing, medication stabilization, suicidal ideation, and grief or trauma. Midway staff assists the individual in gaining independence, self- confidence and self-respect in a safe and caring environment. Services available are defined by the needs of each individual, but may include support in medication management, independent-living skills, referral to community services, individual assessments, and 24-hour supervision. This is a full time overnight position. Overnight staff work four, ten hour shifts 10 PM - 8 AM..

The Mental Health Professional performs professional level work involving the recovery, support and advocacy for individuals with severe and persistent mental illness and/or AODA issues. Coordinates case management with county caseworkers and/or guardians. Assists with the operations of the crisis home.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
This list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time.

  • Researches and designs curriculum for client education on coping skills and daily life activities. The training will be administrated by the incumbent as well as coworkers.
  • Audits client charts on a daily basis to ensure completeness and accuracy. Takes action to fix any errors or incompleteness, either directly or by delegating to Mental Health Specialists.
  • Provides a source of knowledge and direction to Mental Health Specialists. May be assigned a specialty to be the go-to resource for coworkers.
  • Creates treatment plans (ISPs)
  • De-escalates crisis situations using non-restrictive techniques
  • Provides support to individuals, advocates and assesses problems and establishes a recovery focused environment within the individual’s home, group living or community.
  • Establishes and maintains confidential files and records and will be responsible for tracking and reporting on various statistics and demographics.
  • Identifies emergencies and responds appropriately by taking action or directing the action of Mental Health Specialists
  • Works as part of a community treatment team, including internal and external stakeholders
  • Facilitates support groups with an emphasis on recovery and strength based techniques
  • Transports and escort clients to meetings, services, appointments and other activities
  • Attends staff meetings and participates in training activities as required
  • Participates in community advocacy for individuals with mental illness
  • Administers medication, ensuring compliance with prescribed dosages and time-frames
  • Plan and facilitate social, educational, or recreational activities and support individuals’ participation.
  • Promotes an independent lifestyle through skill and resource building and connecting individuals to supports in the community.
  • Assists individuals in identifying and achieving goals
  • Observes, documents and reports clients progress to staff, supervisors and social workers and follows the individualized service plan and/or safety plan.
  • Maintains confidential client information and records
  • Models appropriate and healthy boundaries
  • Supports individuals in making decisions that promote and ensure their safety
  • May administer basic first aid as needed and/or assist with other daily healthcare needs
  • For group living, other house duties as assigned such as grocery shopping for the house and moderate house-keeping including cooking and cleaning. Outdoor maintenance (lawn mowing, shoveling) may be required as well.
  • Ability to work independently and problem solve efficiently.
  • Must be flexible and adapt well to change.

QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION AND/OR EXPERIENCE:
The incumbent shall have at least a Bachelor’s degree in social work, human services, psychology or related degree and a minimum of one year of combined experience providing mental health services OR work experience and training equivalent to a to a Bachelor’s degree including a minimum of 4 years of work experience providing mental health services. Knowledge and understand of DHS 83, DHS 34, DHS 88 and DHS 50 is preferred.


CERTIFICATES, LICENSES, REGISTRATIONS:
Must have a valid driver’s license and have reliable transportation to perform the essential duties of the role; a motor vehicle check (MVR) with a satisfactory driving record per the LSS Driver Safety Procedure is required, and ability to meet LSS auto insurance requirements.

TRAVEL: Ability to travel on day trips as required up to 25-50%, depending on specific role.
