Our Long-term Care Functional Screeners will provide excellent support in administering the Long-Term Care Functional Screen application which is used to collect information about an individual's functional status, health, and need for assistance for various programs that serve the frail elderly and people with intellectual/developmental or physical disabilities. The screen is used to determine functional eligibility for the adult long-term care waiver.
Generally, the position is Monday through Friday, first shift. The position works from home unless administering screens in the community.
Under direction of Program Manager, the long-term care functional screener performs professional work involving the administration of Long Term Care Functional Screens (LTC FS) to participants enrolled in the IRIS (Include, Respect, I- Self Direct) program, in order to establish a level of care and functional eligibility for all publically funded long-term care programs serving adults in Wisconsin. As a member of an ICA (IRIS Consultant Agency) Team, partner with individuals receiving long-term care supports and services to understand and develop what is possible and desirable in their lives.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

- May or may not be an IRIS consultant.
- Establish and maintains effective working relationships with clients, families, caregivers, service providers, volunteers, community agencies and the public.
- Promotes positive communication and support within the screen team and provides consultation with Connections IRIS consultants regarding LTCFS procedures and budgets.
- Administration of LTC FS annually and/or when rescreens are requested.
- Coordination and data collection of LTC FS renewals.
- Provides highly effective support and assistance to enable individuals to understand and meet requirements to obtain and maintain funding for support and services through IRIS
- Continually seeks to learn and to share better ways to enable those we serve to lead more fulfilling lives.
- Works as part of an ICA team to participate in hiring, peer mentoring, learning, and mutual support to others within the team.
- Participates in staff meetings, multidisciplinary team conferences if collaboration is needed regarding eligibility/ineligibility for a rescreen.
- Works to strengthen connections within neighborhoods and communities that may strengthen mutual support and caring among individuals they serve, their neighbors and other citizens.
- Maintains case records in compliance with agency, state and federal standards
- Prepares written reports and may testify in court in evidentiary hearings, contested cases and annual court reviews.
- Insures the proper administration of required documentation for federal and state funding sources.
- Establish and maintains effective working relationships through consultation and coordination with other partnering agencies such as Self Directed Personal Care (SDPC) and Medical Assistance Personal Care (MAPC) agencies.
- Record keeping including regional LTCFS spreadsheets, referral tracking sheets, verification of vital information and screen data.
- Participate in team case assignment and team screen reviews.
- Development of meeting agendas, facilitate monthly LTCFS meetings, take minutes on a rotating basis.
- Participate in training and mentoring of new screeners
- May perform professional work involving the coordination of Long Term Care Functional Screen (LTCFS) team liaison responsibilities with the Department of Health Services (DHS), IRIS consultants and other related agencies in order to establish and maintain high quality screening services.

EDUCATION AND/OR EXPERIENCE:
- A Bachelor’s Degree in social work, psychology, human services, counseling, nursing, special education or a related field or an equivalent combination of education and/or experience is required.
- A Minimum of one year of experience completing LTCFS with seniors and/or people living with disabilities preferred.
- Has and is able to maintain a valid Certification of a Long Term Care Functional Screen.

CERTIFICATES, LICENSES, REGISTRATIONS:
- Must have a valid driver’s license and have reliable transportation to perform the essential duties of the role; a motor vehicle check (MVR) with a satisfactory driving record per the LSS Driver Safety Procedure is required, and ability to meet LSS auto insurance requirements.
- Has and is able to maintain a valid Certification of a Long Term Care Functional Screen.
TRAVEL: Ability to travel on day trips as required up to 80%. Some overnight travel may be required.
