Lutheran Social Services of WI & Upper MI is currently seeking a Long-term Care Functional Screener to join our IRIS Consulting team. Hours for the position are primarily Monday through Friday first shift, but require some flexibility to support the needs of the individuals supported with a strong commitment to enabling people to be in control of their lives.

Under direction of Program Manager, performs professional work involving the administration of Long Term Care Functional Screens (LTC FS) to participants enrolled in the IRIS (Include, Respect, I- Self Direct) program, in order to establish a level of care and functional eligibility for all publically funded long-term care programs serving adults in Wisconsin. As a member of an ICA (Independent Consulting Agency) Team, partner with individuals receiving long-term care supports and services to understand and develop what is possible and desirable in their lives.

Essential Duties and responsibilities:

This list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities as identified by the ICA Team.

  • Develops trusting relationships with individuals served by the ICA.
  • Administration of LTC FS annually and/or when rescreens are requested
  • Coordination and data collection of LTC FS renewals
  • Provides highly effective support and assistance to enable individuals to understand and meet requirements to obtain and maintain funding for support and services through IRIS
  • Continually seeks to learn and to share better ways to enable those we serve to lead more fulfilling lives.
  • Works as part of an ICA team to participate in hiring, peer mentoring, learning, and mutual support to others within the team.
  • Participates in staff meetings, multidisciplinary team conferences if collaboration is needed regarding eligibility/ineligibility for a rescreen
  • Works to strengthen connections within neighborhoods and communities that may strengthen mutual support and caring among individuals they serve, their neighbors and other citizens.
  • Maintains case records in compliance with agency, state and federal standards
  • Prepares written reports and may testify in court in evidentiary hearings, contested cases and annual court reviews
  • Insures the proper administration of required documentation for federal and state funding sources
  • Establish and maintains effective working relationships through consultation and coordination with other partnering agencies such as SDPC and MAPC agencies.
  • Establish and maintains effective working relationships with clients, families, caregivers, service providers, volunteers, community agencies and the public

Requirements

EDUCATION AND/OR EXPERIENCE:

* A Bachelor’s Degree in social work, psychology, human services, counseling, nursing, special education or a related field or an equivalent combination of education and/or experience is required.

* A minimum of one year of experience working with seniors and/or people living with disabilities

* Has and is able to maintain a valid Certification of a Long Term Care Functional Screen.

* Have a strong commitment to enabling people to be in control of their lives.

* Have a strong commitment to support people’s opportunities to live meaningful lives as valued citizens, friends, and neighbors.

* Capacity to support individuals served to meet required IRIS policies and procedures in ways that maximize administrative ease.

* Capacity to work both independently and as a member of a creative, flexible, empowered ICA Team.

* Capacity to use and apply smartphone, tablet and related technology.

* Considerable knowledge of human behavior, dynamics of groups, interpersonal relations, negotiation and social interaction.

* Considerable knowledge of social problems and their effects on individuals.

* Ability to organize and plan work effectively.

* Ability to establish and maintain effective public and working relationships.

CERTIFICATES, LICENSES, REGISTRATIONS:

The employee of this position must have a valid Driver’s License and car – Motor vehicle check (MVR) with a satisfactory driving record required. Must pass a Caregiver Background Check according to program regulations and LSS best practice.

* Has and is able to maintain a valid Certification of a Long Term Care Functional Screen.

TRAVEL: Ability to travel on day trips as required up to 80%. Some overnight travel may be required.
