Menlo Park is located in the Hazelwood neighborhood, just adjacent to Glendoveer Golf Course, near the junction of I-205 and I-84. Menlo Park's knowledgeable and spirited team of care professionals work around the clock to provide the very best skilled nursing, rehabilitative therapy, short-term and long-term care for our patients. The team take pride in working together to ease the transition from hospital to home. Our goal is to help individuals return to their active, independent lifestyle. Specialized services help patients who have suffered an unexpected injury, illness and who are recovering from a surgical procedure. We do it all with heart and compassion.:

Oregon SNF Union Employees:
* Employee Only: $35/month
* Employee Spouse: $70/month
* Employee Family: $105/month
Oregon SNF Non-Union Employees:
* Employee Only: $75/month
* Employee Spouse: $200/month
* Employee Family: $300/month
Our comprehensive benefit package includes:

* Coverage begins the first of the month after start date

Long-Term CareWorks' CNA Apprenticeship a year-long, no cost program that will give you the training and support you need to become a Certified Nursing Assistant (CNA).
The apprenticeship includes:
