Description

Lutheran Social Services of WI and Upper MI is currently seeking a Bilingual (English/Spanish) Family and Community Professional to join our Children's Long Term Support (CLTS) Waiver team serving Waukesha County. The Children's Long Term Support Waiver is a Medicaid funded programs serving children from birth to 21 years of age who have a developmental disability, physical disability, or severe emotional disturbance and their families who have substantial limitations in their daily activities and need support to remain in their home or community. The roles requires regular visits with clients and their families in the home, long-term care functional screening, and a significant amount of administrative work.

The position is primarily Monday through Friday, first shift, but requires flexibility to meet the needs of the client and family.

Essential Duties and Responsibilities Include:

  • Assistance to establish and maintain program functional and financial eligibility (includes initial assessment for CLTS) for children birth to 21 years of age with cognitive disabilities, physical disabilities, and severe emotional disturbances
  • Establishment and reevaluation of level of care for children in the program
  • Ongoing assessment and periodic reassessment of participant health, safety and functional capacity
  • Person-centered, family centered service planning and service plan development, service coordination and plan review
  • Contracting for services: establishing, contracting for and monitoring service systems specific to the participant's individual service plan
  • Reviewing or completing individual service/support plans at required intervals
  • Identify participant outcomes, arrange services; coordinate and manage multiple service providers and between providers (e.g., schools, therapists, nurses, job coaches, personal care workers, volunteers, etc.) to meet individual outcomes
  • Ongoing evaluation of the effectiveness of services and service providers
  • Monitoring and review of participant progress toward meeting service or therapeutic goals and objectives and for Children's Long Term Service Waiver outcomes in service plans
  • Compiling and maintaining required documentation

Education and/or Experience:

  • Through a course of study and practice experience that meets requirements for state certification/licensure as a social worker and also one year experience with the children or
  • Through a course of study leading to a BA/BS degree in a health or human services related field and one year of experience working with children or
  • Through a minimum of four years' experience as a long term support CM/SSC, or
  • Through an equivalent combination of training and experience that equals four years of long term support practice in long term support case management practice, or the completion of a course of study leading to a degree as a registered nurse and one year of employment working with children.
  • Considerable knowledge of counseling and casework principles and methods.
  • Working knowledge of welfare, health, mental health and community resources.
  • Working knowledge of human behavior, dynamics of groups, interpersonal relations, and social interactions
  • Working knowledge of federal, state, and local rules and regulations governing the delivery of social services.
  • Working knowledge of social problems and their effects on individuals.
  • Ability to provide or coordinate skilled treatment to clients.
  • Ability to organize and plan work effectively.
  • Ability to establish and maintain effective public and working relationships.
  • Ability to prepare and present detailed and complex reports.

CERTIFICATES, LICENSES, REGISTRATIONS

Qualified applicants must have a valid driver's license and have reliable transportation to perform the essential duties of the role; a motor vehicle check (MVR) with a satisfactory driving record per the LSS Driver Safety Procedure is required, and ability to meet LSS auto insurance requirements.

TRAVEL:

Ability to travel on day trips as required up to 80%. Some overnight travel may be required.
